I have a medical request and have to do physiotherapy; I would like to do these treatments from you. How can I do that?
If you have only the medical request, to book the sessions you must call or turn to the Cup.
The Usl specialist has issued me an individual rehabilitation project “PERCORSO 3”, how can I access to perform sessions from you?
It can scan the project and the commitment to segreteria@fondazionecrida.it. In the body of the mail you must write all your personal data, including address, two telephone addresses and the name of the basic doctor. Or, deliver the documentation to one of our offices. He will be contacted by us.
How can I do to perform treatments in agreement with your facility?
You have to go to your doctor, he'll be the one who's going to direct it to the most appropriate path.
I finished a rehabilitative path in convention, can I continue privately?
Yes, it is enough to advise your physiotherapist before finishing the cycle, so as to continue continuously. In case the cycle is finished, contact the secretariat that will insert it.

The family specialist/medic, advised me to do privately physiotherapy treatments, how can I do?

In case he doesn't have a medical report, can arrange a visit with our specialists to identify the specific treatment, or an evaluation with a physiotherapist, they will then assess whether to make a specialist visit.
